I've got the best results staying on task by minimizing any potential distractions beforehand, using a ticking pomodoro timer or one that has a visual representation of how much time is left is super helpful as well.
I also recommend taking occasional breaks and getting some energy out by walking or light cardio - you'll be able to focus much better when you return.

If meds are off the table, clean up your diet hard (zero sugar outside of fruit, low carb, high protein, high healthy fats, ideally meat, fish, eggs, light on the dairy, veggies & fruit obviously).
Remove any and all distractions first - a minimalist life works best. Keep a weekly to-do list that is flexible in terms of when you do things; this is to ensure that you can use impulsive motivation while still slowly building discipline.
Hit the gym obviously, switch your cardio to HIIT to stay with it. You might be prone to overdoing it in the gym (in order to get more stimulation), so use a program with a relatively slow progression and stick to it.
Pick a day of the week where you aggressively power through all the tasks that you have a really hard time with.

this hit too close to home. I pushed for so long with the whole boomer philosophy of "just man up, just power through, just toughen up" that I ended up with panic attacks, constant recurring nightmares of insects eating me, randomly collapsing from exhaustion etc.
And it's a fucking joke, after years of "need no meds", to-do lists, steady workouts, controlled diet, constant frustrating attempts at meditation, constant alienation and failure despite doing everything "right", getting on a light dose of meds fixed every single problem I had over the course of a month, and it's stayed fixed for years.
Literally went from renting a shit room and sleeping on the floor to living with my fiancee in a beautiful apartment in the city, huge strides in career & physical health, fixed long standing health issues that seemed insurmountable before the meds.
If anything, I regret taking retarded boomer advice from this place back then.
My girlfriend has adhd but was only diagnosed recently. Before that, she worked around it by very meticulously planning out her day and being very thorough in preparing everything. I also tell her when she hits the rep goal in the gym when we train together because she can’t focus on counting and lifting at the same time
on a psychological level, meticulously planning out can become a problem as most people with ADHD have a tendency to overestimate how much they can do in a day (and with ADHD it's highly reliant on day-to-day form and mental state). This can lead to shame and self loathing for not fulfilling all steps planned for a day. Keep an eye on that.

Be very careful with caffeine, while it can provide a short term push, it can also really fuck with long term concentration and worsen ADHD symptoms (though if you truly have ADHD, most stimulants should have a very calming effect on you, otherwise you've been misdiagnosed).
There are certain long term psychological effects of ADHD, it's good to learn up on them so you can work on fixing them (rejection sensitive dysphoria, tendency to overwork yourself, sleep issues and revenge procrastination).
Finally, I achieved best results by skipping breakfast and getting straight to work on any tasks at hand before I could get distracted. So on wake up, straight to work/onto computer/to the gym with no distractions seems to lead to ideal productivity.
And I can't stress this enough, do not take advice from dumb normies on IST, that's like taking dating advice from incels.

this is out of left field, but for some reason it works for me. Can you take Ibuprofen? When I have to get shit done and sit for like an hour or two, I take 400 to 800mg and it puts me in the zone. I don't know if you can take any caffeine but I usually drink a coke or redbull with it otherwise it can make you drowsy. Alternative ways to stay awake are to go for a walk or jack off a little without cuming
