Pretty sure you can go to an ER and they'll just treat you for free if you have no insurance, you're just gonna have to wait 7 hours

ER isn't free. They'll treat you, then send you a bill. It's only free if you ignore the bill and ruin your credit and deal with harassment from bill collectors the rest of your life.

Just go to your local public hospital
Either that or just foot the 500 yen bill to see a doctor and 100 yen bill for medicine with your national health insurance

    Why is life like this?

I grew up in a very sheltered life, no serious illnesses in my family, grandparents died of “old age” in their 90s.

But I hear other stories, people dying all the time from all this random shit.

Are cancer stars way overblown, or have I just been lucky that I’ve not had to witness any of this stuff with anyone around me?

You're lucky you've never had to deal with it. A lot of it is genetic so if you don't have a family history and people don't do stupid shit like smoke you might not hear about it. Others come from a genetic line where it gets almost everyone in their family.
