The obvious shit is rapid muscle growth that comes out of nowhere despite the person having very little history with fitness/bodybuilding. Along with that you see very specific growth in areas with high androgen receptors like the shoulders, along with other physical changes like acne and hairloss. In women, deepening of the voice as well.
Also increased vascularity due to rapid shedding of fat while rapidly gaining muscle. Which is basically cutting fat while gaining muscle which is really hard to do because to cut fat you need to be in a caloric deficit typically, while to gain muscle you need to be in a caloric surplus. To contrasting actions that doesn't make sense.
With all that said, there are exceptions and as a result you can't ever know for certain save a result for a test. You might be 99% positive, but there are freaks of nature out there.
It is possible that they are new to lifting, for example, and get that rapid muscle gain that accompanies it. Not unusual at all. With that can have the perception the person is roiding when all they're doing is spending a lot of time in the gym and doing all the right dietary things.

it really depends on your physiology, your lifting form (which muscles you load most for certain exercises), and the relative densities/sensitivities of your androgen receptors. By the second item, I mean that for bench press (for example) some people mentally recruit their triceps more than their shoulders, or possibly the other way around. This is where mind-muscle connection plays a big role. But also the density/distribution of androgen receptors does a lot

My buddy who has been juicing for close to 10 years used to say, they can be big (over 6' 2" @ 250 lbs), lean (sub 10% bf), or natty. You can't have all 3. So someone who is super lean and avg size could be natty but someone who is the size of the Rock (6' 5" @ 260 lbs) is almost always not natty. I think the theory holds up pretty well as you see plenty of natty big huge dudes that are not shredded and smaller shredded dudes as well.
Also because the deltoids and traps have a higher amount of androgen receptors, 95% of people who juice get bowling ball delts and huge lats. This effect is extremely obvious on women since they lack very much test to begin with.
Your threshold is way too high. Natty limit at 6’2 and 10% is MAYBE as high as 215lbs.
If they look like a pro bodybuilder in size and leanness, they're juicing (and probably on gh and slin).
Otherwise, no definitive tell. Gyno, bad acne, bald, or crazy sudden progress are tip offs though but aren't 100%
