                Yes. You will encounter things that will make you uncomfortable and anxious everywhere in the outside world. You need to find copes to deal with it not the other way around. One is learning that people may wear some edgy shirt and it's not a direct attack to you, they may not even notice you or care about you at all. For all you know this guy used to be super skinny and to him it's a motivator.

                I'm a mentally ill moron (aspergers) myself tbh and just reciting some useful stuff about learning other peoples perspectives from the DBT program for borderliners.                 The fact that he used to be super skinny and still need to wear that shirt to cope with his memories of him being skinny seems to indicate that he's prone to judge every skinny in the gym. Why? Because he rejects his old body, he doesn't want to be related to skinny people, he needs his shirt to distinguish his old version of him (the one he dislikes) and what he is now. That's my interpretation.

                I would wear some edgy shirt like that to remind myself of my worst as a warning to not let myself go like that ever again. Would I care about other people being skinny? No, because I barely notice people from other objects in my environment.
                People have different degrees of empathy. Some don't give a frick about others, others get too involved into what others think and feel about them. I'm very far on one extreme, you seem somewhat extreme on the other end, because you'd feel anxious over a T-Shirt slogan on some random dude. A mind that looks at other people and only sees criticism of yourself in others is not a healthy mind. There are skinny people out there that don't feel bad or insecure about being that way. Do you think they'd even notice this guy on the street?

                This is not exactly "projecting my own thoughts to other people" because I don't despise skinny people. I despise that aspect of me personally but I'm not judging people by their muscles. My hint is the following : people who go to the gym did start with the same mindset as me, despising their body (either skinny or fat). For those who managed to get rid of their old body and became muscular, there's an important risk: still not being confident despite being muscular. Why? Because maybe lifting doesn't solve the roots of the problem. It may have just shifted it, changed the way the problem is expressed: people are muscular but still insecure because they fear becoming skinny again, so they reject and disdain skinny newcomers (or fat, depending on the case, but I'm skinny so I'm talking about that)

                That's the problem. I can accurately judge the mindset of a friend of mine with whom I would go to the gym. I cannot accurately judge the mindset of someone I don't know. Maybe he's gonna be nice. Maybe not. However, the "maybe not" risk is still present. And I assess it quite high. Would I go in place where I think the risk of being mocked is high, especially if it is a domain 1) I value a lot 2) I'm rubbish at?

                I've never learned how to work like that, and I assume there's a nuance between your mindset (= I don't give a frick at all) and mine (= I care about everything). Usually people don't care, except if someone unusual happens (for instance, if someone stumbles in the street) or if they see someone with a peculiar profile.
