they're supposed to give you an oral medication you take for months. It's hard on your liver so they usually have to get blood work with it. In cases of reinfection if it's painful they can just remove the nail and sterilize the nail matrix.

Tried salicylic acid for 3 years, then covering it in vic's papo-rub for a little over a year, tried a prescription topical (forget the name) for a year, and I've been using tea tree oil daily for the last 8 months.

I recently used to have toenail fungus, not as bad as yours though. I went to a derm and he prescribed me oral Terbinafine. I could see how the discoloration stopped at the bottom of the nail until all the infected nail areas grew out at the top. It took me a total of 7 months to get completely rid of it.
I don't know if Terbinafine will be sufficient enough for your case, or if you need someone to trim down the nail. Either way you'll have to go to a doctor.

No, no side effects. Strictly speaking you are not allowed to drink alcohol or caffeine. But look for anecdotal cases online of people consuming both while taking the orals and experiencing no side effects whatsoever. I skipped the caffeine and alcohol while I was doing the treatment since I am not a fricking addict. My derm checked my liver via bloodwork before and while I was taking the medication.

Is there any point in treating this shit? I have had it on my toes for over half a decade. I keep them short and neat and it doesn't bother me too much.

I did treat it once topically, daily applications for a year, it fricking sucked. Then the shit just came back anyway. So I'm not doing that again.

Then there's the tablets (terbinafine) which have shitty side effects, possibly even including hair loss. And I read many stories about it coming back after stopping the tablets anyway.

So yeah, shit sucks but what are you gonna do? Autistically rub shit on your nails for the rest of your life? Take tablets that stop you going in the sun or drinking beer? I mean wtf are we supposed to do here. I'm just gonna continue with these shitty yellow nails I guess.

      terbinafine

take it for 3 months, consult your medic it can frick your liver if your a pussy, after a year my nail look better than ever

      These are all correct.

OP, Dermatologist here. Go see a doctor, it's the only way. We can remove the toenail (or you can do it yourself) but the fungus will return. The fungus is under your skin, in the nail root and sinus, and so it has to be taken care of with medicine. Everyone uses Terbinafine/Lamisil because it works the best.

I know you can get it in topical, but that's really only used for fungal infections at the surface, like ringworm, jock itch, athlete's foot, etc. It does not get deep enough to affect what's causing toenail fungus. Occasionally I see patients who insist on it, but that's because they're alcoholics. They try it for a month or two, come back to me, and then most of the time choose to live with the nail fungus.

Anyway, the alternative to topical cream that actually works is getting a 250mg pill per day for 2-3 months. You should not drink on this. Terbinafine taxes the liver (I forget exactly how), but it doesn't work as well and alcohol really stresses it out. This can lead to liver failure, which will manifest either as yellow eyes and skin from an excess of bilirubin or darker colored urine because things aren't being filtered properly. Keep taking it for however long the doctor recommends. At a minimum do it for four weeks after you can no longer see fungus to be certain you got everything under the skin. If you stop too soon, it will just come back and you're at square one again.

        Thanks Doc.

What do you think of the multiple layers? I started going down the YouTube rabbit hole and I keep seeing videos of nails like this where the podiatrist explains how the nail bed is permanently damaged so they cut it back to the part that's still alive. Then they attach a fake nail and the patient has to regularly go to a nail salon.

          Anonymous

Again, you need to go see a derma in person to better advise you. Prolonged fungal infection do eventually damage the nail root, causing it to grow sporadically and strangely. I've watched those videos too. Even if the doctor filming doesn't mention it, the patients are absolutely getting put on oral antifungals as part of their treatment.

dermabro, I have a weird vertical dark line on my foot thumb toe, is that a fungus?

          Anonymous

One of two things. It's not a fungus.

First, a splinter hemorrhage. You probably stubbed your toe on something. Not serious

Second, linear melanonychia. The melanocytes in the base of your nail are normally dormant. These activated for some reason and are producing melanin, discoloring the nail. Number of different causes, almost all benign, including stubbing your toe. Only thing to watch out for is if it keeps growing in width or starts getting splotchy. If that's the case, you have a melanoma and it needs to get taken out, as melanomas at that location metastasize aggressively.

            Anonymous

            >linear melanonychia

yes, I believe that's what it is, my toe looks exactly pic related, I have had it for like 8 years. I had this bad habit of fooling around playing soccer on my bare feet, so it could be I stubbed my toe back then. Is there a way to remove it? the dark area hasnt grown or caused any pain so far.

              Anonymous

Nah. If you've had it for 8 years, it's not from acute trauma. What you have is the equivalent of a tiny mole in the basal layer of your nail root. No reason to remove it. Doesn't look concerningly large. Save this picture and use it as a reference every 6 months or so to see if it's widening. If that happens, or if you start getting pain there, go see a doctor.

I've never seen it contraindicated, but I'd get tests done before starting it and then two weeks on it to see if anything bad is happening.

VINEGAR, straight white distilled vinegar. it makes the area unlivable for bacteria/fungus. you can cut it 1:1 with water if you need to. but you can just spray it on or dab with some tissue. do it 3 times a day, that and the surrounding area. can also spray anything your feet come into close contact with where the half life of the fungus might still be present to reinfect.

Stop fricking spamming this. Vinegar cannot penetrate to the nail root and basal matrix where the fungus is. If topical application of the drug specifically designed to kill this type of fungus is unable to have an effect, what makes you think topical application of vinegar will?

Is there a way to get these fungi to produce fruiting bodies (mushrooms)? As I understand it, the fungus needs to grow into contact with another genetically different fungus, so would it work to get two types of fungus on one nail (innoculate nail with second fungus) or bind two peoples feet together for a month?

          Anonymous

Not possible I think without hoping a very rare mutation happens.
Fungi decompose, they do not bear fruit, these are the ones you see here.
Fungi are also mushrooms and they have mutated away from the others and have become less deadly an a result, and have further mutated into yeast varieties.
Only 1% of fungi bear a sort of fruit aka mushrooms.

What you are promoting works only in 30% of cases and fricks your liver in 100% of cases.

Yes, it is worth getting medical treatment so you are not host to a parasitic fungus for the rest of your life. That shit is unbelievably nasty.

        Anonymous

Did you even read my post? have you had first hand experience with this shit?

the tldr is this: the treatments have big downsides and there's a good fricking chance it's gonna come back when you stop anyway. QoL is probably better if you just keep them short and put that shit out your mind.

Side effects should always be taken seriously, yeah you got a low chance, but you never know. I never had any problems with any medications before, then I got some pills prescribed and wind up having some brutal side effects that later on gave me some odd health problems that couldn't really be treated, aside from pain medication. And that's nothing really, some people get their lives destroyed by side effects, always consider that some medication might actually make things worse than they were before. But that's life I guess.

Don't need a doctor. Just need a consistent routine.
I soaked my foot in Hydrogen Peroxide and Apple Cider Vinegar, alternating every day. Hydro. Perox -> Vinegar, and so on.

The Hydrogen Peroxide soak would hurt after about 15 minutes at which point I'd end the sessions. For vinegar did longer around 20-25 minutes.
Every day for 2 weeks and the nail started to die and fail off. Health nail grew back.

Keep your feet dry and watch where you step around barefoot!

      >Baking soda. In addition to treating fungal toenails, baking soda is great for neutralizing foot odor. Mix a half-cup of baking soda, a half-cup of Epsom salt, and one-fourth cup of hydrogen peroxide (3%) in four cups of hot water before adding one-fourth cup of white vinegar. Soak for about ten minutes, and then wash with clean water and thoroughly dry your foot/feet.

I also used this but the nail would hurt decently after 10 minutes.

        Anonymous

Can you just shave down your nail and just apply the peroxide directly onto the nail bed?

          Anonymous

Bro I think that would be extremely painful.
When I was doing the soaking, the nail got progressively weaker as the fungus died. Instead of taking the whole nail off (which doesn't kill the fungus embedded in the nail bed), I clipped the nail very short. Then more and more of the nail came off until eventually the whole thing was off. This was after about 1.5 weeks if i remember right.

Then another week maybe I did soaking and eventual nail grew back healthy.

Again, I didn't have crazy bad growth so it might take longer for other anons but I swear by this method.

No man it was 6 years ago and I didn't real think of it at the time. I was living abroad and didn't want to go to the doctor so I googled home remedies. This came up so I found where they sold Hydrogen Peroxide got that, and vinegar is easy to get anywhere.

I didn't expect much but just kept it up daily. My toenail fungus was not on the same level as some of the people in this thread, so if you have a severe case might take longer.

Mine was comparable to pic related, though.

I suffered this shit for maybe 9 years. Only thing that worked was oral terbinafine, which apparently can frick up your liver, so you must be fairly young, in good bodily shape, and no alcohol, and you can only take it for 3 months. I did see recession in the fungus after 3 months of use but some small part still lingered. I got rid of it all by soaking cottonball in apple cider vinegar and putting it on my toe daily for maybe a month and miraculously it cleared up.
I'll be honest I'm pretty fricking terrified of it coming back so I constantly check my toes and get scared at every single spotty dot on my toenail.
I've heard another method dermatologists use is just pull out your nail and then apply topical cream on the area where the nail used to be. Apparently it's efficient but involves getting rid of your nail and then you will probably have a year or so until a new nail grows

    get that physically removed

then, get a potatoe, a 5 gal bucket, and a handful of leaf litter - the decomposed leaves and soil underneath mats of old leaves, grass, etc. Good compost or biologically active soil will make a good substitute. Boil the potato, mash it, and put it and the water inside of the 5 gal bucket with the soil and a small amount of natural seas salt - less than a small handful but more than a pinch. Cover it and leave it in a sunny place for 1-3 days depending on temperature. Use when the solution is bubbling and frothy at its most vigorous level. Best used up to 12 hours after peak. Soak your toe in the solution. Repeat as necessary.

The chance of topical agents completely eradicating the fungus are pretty low. Especially when the fungus made it's way down to the nail matrix where the new nail grows.
You should get oral medication from a doctor. I think Terbinafine is the most common one.

Not hard at all with an aesthetic impairment like that. But maybe the avoidance of doctors because of first hand embarrassment is a testimony to the shut in, basement dwelling IST archetype. Because if you actually had friends and did stuff except for playing video games all day, the first hand embarrassment of other people seeing your nails would be far greater than the first hand embarrassment you would experience if a medical professional who went to college for such cases had a look at your nail.

had this on my nail since 12 years old
then my hands thumb at 16
then some other nails in my hand at 20ish
im 31 now and almost all my nails in my right hand are becoming rigid/flakey with a slight yellow tint (though most a healthy color to the eye unless you shine really bright light)
i'm on that terbanafine for 2 months and hopefully will stop taking it soon after i get a liver blood test done

it really didnt bother me until they started flaking and cutting into my skin whenever i would accidentally bump my hand or just from dealing with fabric, very painfully they sometimes get caught on something since the ends of the nail become warped and rigid and start to splinter off like wood

yeah idk how it got to my hand, it started on my toenail, but i'm literally a clean freak and wash my hands with soap like 12 times a day
actually i think thats what caused them to get so dry (but i later found out thru the derm that its actually a fungus)

          Anonymous

also i noticed my fingers are infected usually if i use them a lot (like for swiping my phone, scroll wheel on mouse) that seems to break the nail

            Anonymous

and the friction seems to break the nail bed (my theory) and broken nails turn rigid but still not yellow and a healthy color, then over years they get infected and become yellow which only is like 2 or 3 nails atm (1 of them the toe)

Seriously how the FRICK do you Black folk let it get this bad. I'm a fat frick and worked for years on my feet, all you have to do is not wear anything too tight and cut the tip once in a couple of weeks. Jesus christ.
