      The number one problem is accessibility of high energy food. Obesity indicators are lagging indicators because you don't become obese from one lil Debbie binge. You get it from weeks that become months that becomes years of overfeeding and energy excess. Other than Trans fats and low exercise, the other things you listed aren't causal to obesity, they are side effects to the accessibility of food and bloating of the food industry.

        The old American food pyramid had carbs on the bottom. Everyone got fatter, America just did it a little faster. Probably because the US is an agricultural and food science powerhouse. That's why there are thousands of McDonald's, pizza Hut's, and burger kings in the UK, but no Greggs in the US.

          The reason is not why you think it is. It's because most European nations literally just don't care about fast food chains. There just are none. If e.g. Italy cared about them, you'd expect them to have a fifth of the number of native chains, and individual fast food joints, as the US.

          Yes and no. The cheaper option uses cheap ingredients like HFCS in order to stay the cheaper version. There are infinite foods in the US with normal and elite ingredients, but you decided to buy the cheaper sausage. People who are not concerned about health but their limited means will buy the cheaper food option when available to them.

    One of the main reasons nobody talks about is... Overwork. People work more and have less down time at work then their predecessors. Technology made us restless, and as a result after a long stressful day just getting fast food seems like the easy way out. Interestingly Asia doesn't have this problem because their cheap and easy food options are low calorie.

If you ask me besides soda, sugar in ketchup and bread was one of the major catalysts for the whole packaged food industry to start dumping cheap sugars into everything.
