      Kaioken, as differentiated from a Saiyan transformation, is a high-risk high reward technique, not a transformation. It actively burns through the user's vitality in exchange for an artificial multiplier of strength. it's not a testament to will, and is more akin to tapping into adrenaline to temporarily achieve superhuman feats, at the cost of your physical form. It's why Goku just stops using it upon achieving super saiyan, and training his form to slide the scale so he doesn't have to use KK.
      Its a shitty last-ditch technique, akin to popping epinephrine to overcome a seemingly impassible obstacle.

          he uses it on namek against Frieza
          they even drop a black pill mid arc: they suggest Goku should start using the kaioken, but Kaio-sama says that he is already using it to keep up and it's not working (Goku then uses a 20x and pummels Frieza for a short period)

          he does conserve his energy and only unleash it when he needs it because it's a smart thing to do (even real fighters are taught to do this while fighting), but as the kaioken is a specific technique, it's always unambiguous when he's using it.

        Goku never mastered the kaioken like he did with the super saiyan form,im sure if he kept using it he would have learned to conserve more energy doing the kaioken,even original ssj1 was suboptimal and ssj3 was an energy drain. One of the biggest flaws of dbz is how their constantly ditching techniques and transformations for what its new just because its new and "cooler".Besides that because of the multiplier effect the kaioken had a great power scale curve especially when combined with super saiyan ( which goku later learned to do in the series,although i hate super).

        He did a bunch of times,besides because of how much of dbz battles are not seen because of the hyper speed its possible he still uses it in his base form.
        Kaioken Is supposed to be a temporary boost,not used to exhaustion like he did in the saiyan saga but then learned,if he learns to conserve energy it shouldnt be a problem.

        This,the more akin thing to a kaioken would be to recruit maximum energy for any given short duration intensity like a sprint at 100%, a one rep max and the likes. In the chemistry sense without tying it 100% to fitness it would be adrenaline.

      he has to make physical contact and it takes a while, so it's not that practical

      there are a few unexplained 'magic' things people can do. goku and piccolo have made objects levitate before, and piccolo (and kabito) and conjure up clothes for some reason
