Theoretically you can maingain for a while but in practice my lifts only started going up when I embraced the bulk. You can always cut later and you WILL be bigger when you do if your lifts have gone up significantly. Just don't expect miracles without PEDs.
Generally, yes, you have to gain weight to gain muscle
Once you're late intermediate, you can maybe recomp or even gain when losing, but you'll likely give up before you're advanced enough to not need to gain weight

No. If you are 300lbs and you start lifting and you reach 200lbs bw and lift 1/2/3/4 for 6-8 reps you are saying that you didn't gain muscle?
In most cases assuming you're not morbidly obese yes.
