Has anybody else had their doctor tell them something similar? It’s quite frustrating since that means I wont be able to put on muscle until I gain extra pounds of fat. I’m guessing they want me to reach a healthy BMI before I can exercise again at which point I’ll have gained 30 lbs. Which then brings me to my question of whether I should lean bulk or cut?

what sort of exercise are you planning on doing, if it's just cardio then yeah, listen to your doctor and gain a bit of weight before doing heavy cardio
if you're planning on lifting and gaining muscle, then do it whilst gaining weight, which will mostly be muscle mass
doc is probably assuming that you're an anorexic moron who thinks they can do a half marathon each day whilst only eating a few sticks of celery, which might be why you're in this position to begin with, especially if you've been hospitalised

Bodyweight exercises and a high protein dirty bulk so you put weight on fast, then switch to weights once your arms are no longer toothpicks. Yes you'll gain body fat but you'll also gain muscle can deal with that later down the line once you're not built like a 13 year old boy.
