medicaid only cares about income (mostly, but your ass definitely doesnt have enough assets to worry about). food stamps are a lot stricter, but you can also just use food banks to fill the gaps since food banks are just happy someone comes to take the food. you do need the previous year's tax returns for medicaid however, but since you're broke as hell you just google 'irs free tax file' and use something like freetaxusa.com.
they handhold you through the process (putting 0 for income is a valid input) and once you have that completed return, just go to a hospital's financial assistance department and say 'hey i need help filing for medicaid and im new here.' bring a printed copy of your tax returns and be ready to say youre broke and dont make money; odds are theyll get you approved.
if not, just go to the ER when youre sick and tell them youre broke as hell and theyll 99% of the time wipe the debt clean after you talk to them about it (note: this is only for people making sub-30k (aka 200% above the poverty line); if you make more than that you risk not qualifying and being stuck with relatively fat bills)

You get a gym membership today. You ask the staff to help you get started. You workout full body, try out the machines.
When you get home, you shop for healthy groceries. You are going to bulk the rest of the winter on chicken and rice. Ground beef or salmon. Whatever you can afford.
You are going to sleep early, get 8 hours of sleep.
You will keep doing this everyday. On rest days you do cardio.
If you are able to push through the winter you will see a light at the end of the tunnel, if you give up you have a nice day.
