It depends on how old OP is. If OP is sub 30 likelihood of a heart attack is near zero unless he's 300+ lbs. Granted if you are having legitimate issues see a doctor and make sure you get normal excersize while they are running the heart monitor.
I’m having the same issue. Something I have been trying to keep in mind is that there ar e genuine fat fricks, people over 350lbs, who do just fine. There are absolute fat butter golems out there going about their day walking around all out of breath and they’re not dropping dead. There are fatties like that who turned it around and did fine. There are fat people who smoke 2 packs a day and take in a ton of caffeine and they’re fine, even turn it around sometimes.
There are guys on grams of gear a week also taking DMAA or whatever that crazy pre workout stimulant is called, stacking it with caffeine and they’re fine. There’s dudes on grams of gear a week who are a brick shit house like 250 lean, taking the stims too, also smoking. And they’re fine.
There are literal junkies combining meth and coke and they’re fine.
Sometimes these junkies get tazed by police in the middle of their high and they don’t die.
There are electricians, fat fricks who smoke and drink often in pretty bad shape, who will get shocked and be fine.
Gradual exposure. Keep damaging your heart bit by bit to normalize the feeling, then eventually you can experience full-blown heart attacks without any fear. This method is effective for all phobias.

The notion that your will have a heart attack when you work out is -likely- a delusion, as so that sutta is relevant for how you should attend to your thoughts and the themes therein. If it -is- a delusion then to give it inappropriate attention is unskillful.
I say "likely" and "if it is" because I don't actually know you and I've heard of mention of occurences where people had that kind of obsessive thought pattern and they actually did have something wrong in precisely the way they thought, but I've also heard the opposite, where it was effectively just psychosis or ocd.
You have a fear of death. Know that you will die and, in terms of your perceptions, whether you die now or in 50 years after a long happy life doesn't actually matter, everything ends and you percieve nothing in both circumstances regardless of what you did. All that matters is the present moment, not the past and not the future. All you have is the present moment, the past has come and gone and the future isn't here and hasn't even been determined yet. You are sacrificing the enjoyment of the present moment (your entire life) with fears of the future. All the joy you could receive from working out, or whatever activity you happen to be doing is destroyed in service of a future that doesn't even exist. All you have is right now, and right now you aren't having a heart attack, so cherish that.
If you do get a heart attack, the gym is probably the second best place to have one. You have a bunch of people medically trained for injuries, AEDs on every wall, and people strong enough to carry you to safety. Besides, you'll probably survive.

I wear a heart rate monitor but I only need it during cardio. Helps me stay in the optimal range and no fear of getting a heart attack.

Do a shit ton of cardio to decrease your resting heart rate so any spikes are less likely to give u a heart attack
