Only downside is dry skin so its best to take low dosages of it for a longer period of time rather than large dosages for a short period of time in order to minimize side effects.

The good news is that if you doctor isn't a fricktard and actually titrates the dosage while monitoring blood levels, you are unlikely to have issues. don't listen to this piece of shit. Accutane is very safe but you need bloodwork monthly to make sure it's not fricking you up. (Example: on the highest dose that my derm worked me up to, my triglycerides spiked, so she dropped me by 20mg and all was well again). Go to a derm, it'll be a $20-60 copay, and with acne that severe she will guaranteed get you on Accutane right away. I was on it for 8 months, best decision of my life. No hair loss at all, just had really dry lips (moisturize often and sunscreen everywhere too) and some minor fatigue for a few hours after taking the pills.
Pills cost me $10/month on insurance, bloodwork was free every month, and the monthly derm followup was just my copay. I spent less per month than I did on all the OTC products that don't do shit.

The issue with attempting to sort out a health ailment is every interaction you have with the material world can impact your health in some way, literally everything. There are thousands upon thousands of biochemical reactions going on in your body right now, so many different nutrients play roles in so many different things that you'd think aren't even remotely related, but they are, so trying to boil it down to one singular factor is probably futile, and it's why every thread about X health issue devolves into a roulette of suggestions/possible solutions. It's the dairy, it's the sneed oils, it's the gluten, it's the carbs, it's the fat, etc, etc.
Fact is, no one really knows. Something may work for someone, but not for you, that's just how it goes. But all that being said, I'll give my 2 cents.
Have you tried eating liver? It's chock full of bioavailable vitamin A. I've read vitamin A from plant foods (the inactive kind) on has a conversion ratio of like 12:1 or 10:1. I.E. if you get 100% of your vitamin A from plant foods you'd actually only truly be getting a 1/12th or a 1/10th of bioactive vitamin A, so many people may be running on the lower end without even knowing - at least that's what I've read anyway. And most acne medications from the dermatologist are just derivatives of vitamin A . Also, zinc, collagen, vitamin C, and copper all play a role in skin health too.
Doctors are only there to medicate you. They suck fricking dick at addressing root causes, and I've had plenty throw up there hands at my health issues and bluntly say, "we don't know". They're not as smart as you think. Any autist with a good Adderall script and unfettered access to google scholar/pubmed arguably knows just as much as them.
accutane before it's too late(scars) i had a worst case, it was bad like yours at first but derms put me on antibiotics(i was too young to know better and refuse) to treat, some miraculous improvement for 2 months then it came back 2x worse than before, cystic painful and keloid scars(and im not even a nig/chink, still got keloids)
accutane 90% fixed it but 10 years later i still get some. had to get hydrocortisone(i think thats what it was) injections for the scars to flatten them, my back still looks like that of a torture victim but niwhere near as bad as it was

I cleared my acne on my legs by applying a skin cleaner and then moisturizer every morning and every evening after showering. I did this for roughly 3-4 months and it fully disappeared and hasn't returned, despite me not applying it there anymore. Basically it cleans the oil off your skin, which causes acne. However my case was not so severe, yours seems more hormonal so I'm not sure it would work for you. Also this is very hard to do on your back due to reach issues.
Other than that Accutane is the only solution imo. It can potentially cause your skin to be drier, but it should take care of the problem. I don't think it messes with hair and in most cases the skin shouldn't be too horribly dry, unless you already have dry skin, which is unlikely. All the people that I know, who took it, are happy with the results. Also during the 6 months or so that you're taking it, you can't drink alcohol, as the medication temporarily frickswith your liver while taking it
Wear loose shirts and change them frequently and never wear the same shirt twice. If you wear undershirts, get rid of them.

Hey OP, I've got the same problem as you, maybe even a little worse. DHT is the culprit. DHT causes hairloss but it also causes increased oil production which is what is giving you that kind of acne. I started losing my hair and hopped on finasteride and my acne vanished. If you haven't already thinning then it would do you good to hop on finasteride.

I'm on accutane right now (60mg daily) and it made mine go away really quick. Just do it man. I tried all of those methods and they didn't work for me. I got so fed up with all the bloodstained tees that I eventually caved in and got accutane but I don't regret it one bit. It's also helped with the acne scars on my chest and face as well, but I'm going to do an acid-based facial treatment once my accutane cycle is finished to remove even more scarring.

Okay Black person, here it is: OCEAN WATER. I've always had bacne but it hasnt been terrible until I started frauding. Then I moved to the beach and spent most if my time splashing in the water and in the sun. Sun + salt water eliminated my acne completely.
Don't live near the ocean? No problem. Buy a bag of sea salt from Amazon--like the kind you'd use for cooking. Mix it in glass bottles to a concentration roughly approximating the osmolarity and ratios of salt water which, in my case, was 3 teaspoons of sea salt per 500 mL water. The key mineral components of salt water are Na, then Cl, then Mg, then Ph. The Mag and Phos are trace amounts and you can probably just use tablesalt bur whatever. After you shower and/or in the AM spray the sea salt shit on your skin and just leave it let it air dry. If you add it to hair it gives you fabulous curls but be careful because its drying as frick. That's it.
My theory is that human skin evolved in an environment of constant sunlight and constant salt exposure via sweat. Most hunter gatherers would not have been bathing regularly, and even if they were they would not soap and they would end up sweating over it again later. Salt is an astringent and antimicrobial.

Problem with bacne is there are a lot of different causes so no one can tell you exactly why you're like this. It's why you get so much conflicting advice and people telling you different shit worked for them.
One thing I don't hear mentioned enough is how hot are your showers? I used to have bacne, not this bad, but I tried a frick ton of shit to get it cleared up. Turned out it was the scalding hot showers I would take. Would dry the frick out of my back, which would then create a porous, dry, rough surface that was prone to infection.
If you take really hot showers try switching to lukewarm and see if that helps.
