Taste is tuned to molecular composition. If something tastes sweet, it's likely your body has a metabolic pathway that can break whatever tastes sweet into energy.
Aspartame for example breaks into an amino acid, wood alcohol, and another amino acid. Wood alcohol isn't good for you. It turns into formaldehyde when it reaches the liver. The two amino acids, phenylalanine and aspartic acid can be plucked apart for energy in the urea cycle.
Sucralose is odd. Studies say it isn't biologically active due to the molecular structure's resemblance to sucrose, but with chlorine atoms attached. When the body encounters sucrose, it's cleaved to fructose and galactose. When the body encounters sucralose, the enzyme meant to cleave the molecule, sucrase, is not able to interact with it all that well due to the unexpected and rather large electronegative properties of the chlorine atoms attached to the molecule. It is said to be mostly excreted since the body can't take in raw sucrose/sucralose without it being broken into monosaccharides. With this in mind, it's worth noting that enzymes are catalysts, meaning they bring the activation energy of a reaction down-- meaning more quick reactions. It does not mean a reaction cannot occur without enzymes, which in the case of sucralose, this is likely the case.
Sucrolose can be hydrolyzed without enzymes, and it can be broken by the gut biome. The resulting chlorinated monosaccharides are then able to pass into the body to be metabolized.
How the dicks a chlorinated galactose or fructose molecule is metabolized is not well studied for -some reason-. However, the resulting molecules of metabolism would likely be an organic salt of chlorine and the two sugar molecules which could be broken down to pyruvate.
tl;dr for sucralose is basically, since it isn't read as a carb/sugar in the nutrition label, it can be heavily loaded into whatever it's sweetening, meaning it can become frickloads of sugar.
This and most sweeteners (doesn't matter if it's zero calories or not) spike your insulin. Which will frick you up really hard.

Artificial sweeteners are some of the most well tested food substances in the world. There are literal billions of people who consume artificial sweetened drinks like this shit every single day for decades, so any negative effects are at absolute worst so minimal they can't even be detected
