The bruise may have been caused in a separate incident, either in his destruction of the bathroom or something unrelated earlier that week (like a dropped weight or bike/scooter incident). If it was caused in the same fall as the face injuries you'd think it more likely that he tripped from the shower recess over the toilet or other debris and slammed his face and thigh into the ground at the same time. Problem with that theory is that he appears in a kneeling position with his thighs off the ground (until his body is pushed over onto its side by opening the door) which would make a ground strike to the thigh difficult to occur in the same action that he's KO'd, unless the first face strike was against the door and opening the door is what pushed him back into the kneeling position seen, but that doesn't match the pooled blood on the floor nor the lack of blood on his body, it looks like his face smashed the ground in the strike and stayed pressed against the floor in a kneeling position.
What looks solid is the head smash -> knocked out & massive bleeding from nose - > face compressed into the ground in kneeling position -> drown while unconscious in pooled blood/blood flowing back into the upper airways from nose.
