It depends on what you want to get from it. Doing it like pic related, where you ease up your mind and just focus on your breathing, how your body feels, do visualization, etc. Will not make you more muscular but will improve your concentration for a longer time and will help you stay calm for the things you train to stay calm on. Anotehr form of mediation is repeating a feeling or a thought so it becomes second nature, like cursing under your breath everyday about how much you hate your job, co-workers, body, etc.

It’s basically like going to the gym for your mind. You’re supposed to focus on something that is going on presently like your breath going in and out or how your body feels in the moment. Every time you notice yourself getting wrapped up in your thoughts and going back to your breath it’s like doing a rep for your mind. It’s all for the goal of becoming more present which ultimately increases your focus and lowers your anxiety. Increases your focus because you get better at noticing how easy you’re getting distracted and coming back to whatever you were doing and lowers your anxiety because being more present means you’re not getting wrapped up in the past which you can’t change or the future which is unpredictable. Not saying you just stop thinking at all, but you’re not letting incessant thoughts rob you of enjoying the present moment. It’s the only time that matters and the only time you have control over.
Adding to this, there are different types of meditation depending on your goals and how esoteric you want to get. Like when I do nofap, part of my meditative practice is me trying to "channel energy" from my balls through my body. Like it can be viewed as a focusing technique or de-escalation of heightened feelings, but I'm just saying that it doesn't have to be "just focus on the breath"

I would also add that losing focus or doing it well shouldn't be something you care about. It's just another feeling or thought that came to you and should be "ignored" rather than torturing because you are doing it badly or held in high regard because you are doing it well. Those are emotions, they cosume energy, letting them go is an efficient way of saving energy that can be used to focus on the breathing or somethgin else throughout hte day.
Not entirely. From what I know of both Christian and Zen practice, the state of consciousness you're working to achieve when you pray (hesychia) is different than when you meditate (mushin). Hesychia is very "I-Thou" affirming, while mushin is "I-Thou" denying. Prayer is designed to build a relationship with God through wandering in the "heart", while meditation is used to enter into a state of "empty mind" often realized through boundary dissolution while being opposed (in kendo's case, combat). In practice, they go about reaching these states in different ways. That said, both are very difficult to understand.

Meditation is at least good at speeding up recovery and has numerous physical benefits even if your not doing it for mental benefits
iirc some monks can train to only need 4 hrs sleep because of their meditation routine.

To mediate is just to think deeply about something, to practice and repeat a feeling or a thought, to heal back up our atrophied minds and counteract decades of recieved stimuli without us giving any meaningful feedback.
