It doesn't have to. If you use a decent enough resistance you have to assist with your arms and that's just going to rocket whole body glycogen depletion.
I work up a sweat when using this thing more than just about anything else I do when it's maxed out. You should be able to adjust resistance and incline.

I use this, it's a pretty convenient way to do my HIIT. Easy on the joints and I have back issues so I need it to be low impact. Also sessions are shorter than if you were to use a stationary bike. Also I don't really get the criticism, it's not like you would expect hypertrophy from doing this, it's just for cardiovascular health and as long as it gets the heart rate up to where you want it, it does exactly that.

Depends on the machine but "incline" on an elliptical usually doesn't increase difficulty it just changes the motion and works different muscles.
