What can I do to fix this? I feel my skin alone takes at least a point of attractiveness off, I'm 22, why is it still like this?

I've heard people say it's caused by hormone imbalance from insulin resistance.

Is it possible for someone to be diabetic (t1)
      without knowing all their life?

No you'd be dead. Insulin resistance is pre-beetus and causes over production of testosterone which causes adult acne. Also sugar cravings, hunger after eating, blurry vision, fat around the midsection, depression, etc...

ok, I'm a little paranoid I have it since I got my T checked a while ago and my free test was quite high (226pg/ml), T was also fairly high (770ng/dl) but I've heard high free test is a sign of beetus, though it wouldn't make sense since I'm quite lean.

thanks for advise, I htink I eat fairly well, less sugar maybe. I've tried face washes meant as acne treatments (salicylic acid, etc) and haven't seen an effect. I'm wondering if I should see a doctor, noone my age seems to have this and I hate it.

literally just wash your face with this. Warm water wash your face then lightly scrub this stuff on. Leave it on there for a minute. Then rinse it all off. Do it once or twice a day- one in the morning and before you sleep. Realize pores get clogged and you need to constantly cleanse.

IST will call you a shitskin but you might have to lay off dairy. I can eat/drink buckets of dairy without intestinal issues but my skin gets super fricked up from it. One day eating dairy products takes like 1+ weeks before the adverse effects wear off on me

Quit refined sugar, try to eat only the healthyfats, eat your broccoli, arugola, quit the milk products.
Wash your bedsheets regurlarly, pillow most important. Clean your body before going to sleep so you have to wash bedsheets less often.
    Get some sunlight.
Use face/body products that have hyaluronic acid.
    Collagen peptides.

Remember your body has to get rid of all the garbage, so it will probably be slightly worse at the start.

Bonus points if you become a warrior against processed foods.

What are your thoughts on skin products effecting the skins natural biome?

Stop eating refined sugar, what kind of soap do you use, moisturize face, face cleaning routine, don't drink lactose milk. I had acne into my thirties. I changed all of those and it's pretty much gone. Soap and milk were the biggest contributing factors. Don't let your face dry out even if it feels like it should fix the issue.

Unironically stop washing your face so hard.
Rinse your face with water in the shower every day, but don't use any sort of soap or cleaning product, it dries you out and makes you more spotty.

Get a popular moisturiser, check out reddit for skincare, find one that's just considered good and start using it twice a day.
I used to be spotty like the OP pic, but stopping with soap on my face and wearing moisturiser every day means I get one spot a month, maybe.
