          Genetics determine how strong/resistant your enamel is, how acidic your saliva is which can either aid or inhibit the growth of cavity-causing bacteria, also the shape of your mouth/teeth which can make flossing difficult or create more spots where bacteria can grow.

    I got a few cavities like this. I didn't start brushing my teeth until I was in my 20s because my parents never taught me to. For some reason all my cavities are symmetrical. My back two top teeth are almost completely gone and I've read wisdom teeth can cause this which I have.

    Anyway, none of them hurt except one of them but I've been ignoring it for years now. If the pain gets too bad you can buy temporary filling putty that will fill the cavity for about a week until the putty breaks. You're not supposed to do this since it locks in the bacteria but they can't stop me. Just clean your mouth good and fill it. You can get 5+ fillings out of $3 kit so theoretically you could just fill your cavity repeatedly with temporary filling pretty cheap.

        Is there no emergency dental work in the states, if I'm not mistake hospitals will treat you even if you're penniless?
        Here, even if you're broke they will fix your teeth if it gets bad enough since its a serious and even immediate health risk and lowers your quality of life hugely.

      If it's just on the surface, a good brush can get it off. My israeli dentist told me to brush more gently and I was stupid enough to listen, so I got some black on my teeth for the first time ever. I caught it pretty quickly and brushed it all off. No issues with it months later now
