Why is my boyfriend morbidly obese when he only eats around 1,700 calories a day?

He's 5"10 and 365 lbs. I live with him, so I can keep track of what he eats. He's been obese since he was a toddler. He doesn't have a thyroid problem, and all his blood tests are normal. Health wise he is perfectly healthy and doesn't have any health issues related to his weight. His blood pressure, sugar and cholesterol are normal. He has three brothers who eat more than him and are shorter and they are very skinny. He doesn't eat breakfast most of the time. I checked the calorie count of the stuff he eats throughout the day and he's eating around 1,700-1800 calories a day. According to an online TDEE calculator his basal metabolic rate should be above 3,000 for his weight.

Do you also track what he drinks? For example 2 litres of coca-cola has 840kcal. Do you track stuff like oil and butter that's used in cooking? Are you together 24/7 and you 100% know he isn't eating above this number?

While there might be a rare disease making him gain weight, it's more common that the tracking is done incorectly or the person straight up lies what he eats, in case of binging disorders, that might be because of shame.

This. My cousin was obese for most of her life. She was tracking her calories daily, even worked out 1-2 times per week but would not lose weight. She was so desperate and frustrated. Then she lived with us for one month, because she didn't find an apartment and it became very clear why she didn't lose weight. She estimated the weight of many of the things she ate, she often forgot to add the calories of oil to the calorie counting app, she often forgot to add drinks and smaller snacks. She highly underestimated the calories of many food items, especially fatty and sugary things. She thought she consumed about 1800 kcal/day, but we figured out it was on average 2900 kcal/day. That's such a crazy difference.

While this is certainly true for some people, I haven't been able to lose fat, nor weight in general, while eating ~2140 calories per day (everything weighed, not estimated) at 6'6, 285 lbs.

My daily macros at 2140 calories are:
        184.45g protein
        168.25g carbs
        96.65g fat

I've eaten this consistently for about a year - before that I ate even less. For the past couple of weeks I've added an additional meal of scrambled eggs, and I've neither gained nor lost any weight in that time.

So your only response is, "You must not be measuring accurately." With how much b***hing and moaning there is over eating chicken and rice, I can guarantee that I eat cleaner than 95% of IST.

What does his primary doctor or nutritionist say?

Surely his doctor/nutritionist is concerned especially there merit to what you're saying?

If you're talking to me, I was diagnosed hypothyroid around three years ago. I later lost my health insurance and have not seen a doctor in about two years.

                >Unless the nutrition labels are wildly off, that's an impossibility.
You are married to numbers and expect accuracy and precision and it is not warranted. The fact that you made no changes in the the last year except maybe measuring even more, super-duper accurately instead of lowering the amount you eat demonstrates that.

                >instead of lowering the amount you eat demonstrates that
In a previous message I mentioned that I ate fewer calories in the past - that's when I was diagnosed hypothyroid. When I increased my caloric intake to ~2140, I did not gain any weight, but had more energy to work and exercise.

I consumed fewer than 2000 calories per day, long term, which led to me going from 230 to 285 lbs. In that same period, I developed gyno, edema, and diffuse balding (not androgenic alopecia). My weight stabilized when I began consuming more calories - not fewer. I predict that I will lose weight as I increase my caloric intake, plateauing at some point at which I'll actually be able to manage a caloric deficit/caloric excess as needed.

if you really are weighing your food and actually counting everything including oils, sauces, and even seasonings like garlic and onion powder, as well as developing even more potential symptoms, then you really should just go see a doctor and get all the relevant bloodwork done and go from there. could be something as simple as low t or any number of metabolic diseases.

Recalibrate your food scale and double check the caloric content of everything you're putting in your mouth. If you're still gaining weight on smaller portions and losing on larger ones (same meal composition otherwise), give some thought to who has motive and opportunity to tamper with your food.

Unless the nutrition labels are wildly off, that's an impossibility. Like I said before, I weigh 100% of my food - I do not "estimate" anything - which is why my macros are listed down to two decimal places.
            >an overestimation of physical activity
My theoretical BMR (i.e. the number of calories I should burn independent of any exercise) is higher than 2130 calories, so that's irrelevant. I could estimate that I am completely sedentary (which I'm not) and it would still contradict this.

I think a problem is that even if you are tracking everything rigorously there's measurement errors already built into the system. The nutritional facts about food aren't always precise and can be off by a certain amount. For example, we keep creating new breeds of fruits and vegetables which alters their macros. The variance between corn is fricking wild, but they just slap on the official USDA database value that was measured who the frick knows when and call it a day.

          Anonymous

IIRC you're allowed some insane level of variance on your calories/macros on nutrition labels, like 20% in either direction. I've seen anons recommend that if you're trying to lose weight, add 20% to whatever the package says, and the opposite if you're trying to gain weight.

        Anonymous

So what? If I underreport my calorie intake by 10% and underreport calorie expenditure by 10%, no harm. If I underreport intake by 20% and over report expenditure by 20%, I'll wind up a fatass if I ignore what I see in the mirror every morning.

Listen I’m not morbidly obese. I’m 5’9” and 225, which is obese.

I’m doing better at the moment, steadily losing weight, but..

I’ve stopped drive thrus, used cash, and then stopped at gas stations to throw wrappers away.

I’ve gone out to get pizza for my family and stopped and eaten fast food on the way, AND candy bars at the grocery store next door while waiting for the pizza to finish.

I’ve hidden boxes of Nutty Bars and Oatmeal Cream Pies in the garage where I knew my wife wouldn’t go.

I’ve gone in the house during family barbecues to “use the bathroom” and eaten cookies and chocolate before coming back out.

I’ve stopped and eaten 3 dollar menu burgers on the way to going to lunch with my boss because I didn’t think the burger I was going to get with him would fill me.

Your boyfriend most likely doesn’t have a magically slow metabolism. He likely has food addiction/ binge eating disorder.
